Referral Program Terms and Conditions
Eligibility:
All approved participants for Cortiva Institute will automatically be
enrolled in the program through their successful completion of their new
student referral. A successful referral is defined as a new student that
enrolls at a Cortiva Institute school in the Massage Therapy program
as a result of the referral made
by a current student attending classes, an alumni or a staff/faculty member,
who remains a student a minimum of two weeks. Students that start in the
same session as the person they refer are not eligible for a referral
reward.
Eligible Activities:
All referrals that result in a new student enrollment whereby the referred
student stays in school for at least two weeks will be awarded. Multiple
referrals for the same prospective student will be assigned based on the
individual who first submitted the referral.
Awards:
Participants may not make any substitutions, including cash, for any awards
earned in this program.
Liability:
Cortiva Institute is not liable for any injury, death, or other loss,
cost, or damage or claims by participants related to the redemption, receipt,
or use of awards in this program.
How the Program Works:
Participants submit referrals by entering data into the referral website.
The system captures the web referral, and generates an email to the referred
student. All referrals must include the name, address and valid email
address for the referred student and the referrer. All referrals will
be kept for 24 months until either accepted or denied. If accepted, an
award will be sent directly to the qualifying referring referrer after
referred student has enrolled and remained a student for two weeks (this
could be up to a maximum of 60 days).
Earning Awards:
Awards are limited to one gift per year and a maximum of $100 of value
per referring individual. Awards will be provided only after the referred
student has started class and remained as a student for two weeks.
Redemption of Awards:
Referral awards are provided after the referred student has enrolled in
class qualified as a student and has attended classes for at least two
weeks. Upon acceptance of the referral by Cortiva Institute, an award
package will be sent to the referring individual.
Taxes:
All federal, state, and local income taxes earned in this program are
the sole responsibility of the participant.
General:
Cortiva Institute reserves the right to alter, amend, or cancel any or
this entire program at any time without notice. Cortiva Institute reserves
the right to interpret the rules of the program in any of their applications.
All decisions on program rules and participant eligibility are made solely
by Cortiva Institute and all decisions are final and binding.
